We've Got You Covered!
Since TSASRDC, Inc. is a member of USDA (United Square Dancers of America), each of our member clubs is entitled to purchase insurance (commercial general liability and accident medical) through the USDA's group insurance program.  According to USDA's insurance brochure, "Accident Medical insurance helps protect club members from financial loss due to a covered accidental bodily injury.  Liability insurance protects the club and its members and association officials from financial loss due to unforeseen incidents which may develop into litigation against members and dance organizations."

Things to Know & Do If You Already Have the USDA Insurance

First, your insurance will become effective on January 1 or the date your enrollment form is received by the insurance company whichever comes later.  (So enroll early to be sure your insurance is effect at the beginning of the year.)

Be sure to keep your club roster up to date.  Send additions to your area's insurance chairman  as they join your club or class.  While class members are free, you must submit payment for any club members you add prior to July 1 of each year.  Download a special form to list the additional members and submit their fees.  No fee need be submitted for club members who join after July 1.  Either way, dancers are not covered unless you have added them to your roster and sent their name to your area insurance chairman.

Before each dance inspect the premises for dangerous conditions such as broken steps, lights out, etc. which present a hazard.  If any are found, complete an Inspection Form and have it signed by the building custodian.  Give the custodian one copy and retain the other for your records.

When a club (after receiving an insurance certificate) has a dance at some facility other than its regular dance place, an insurance certificate will be issued only if required by the owners of the facility.  In most cases the facility owners will accept as evidence of  insurance, the certificate held by the club for its regular dance facility.  The insurance company must be notified even when a certificate is not required.  Send a Notification of Event form to Elmer & Lottie Jackson at the address below with sufficient time for the form to be filed with the USDA by Elmer & Lottie.  Use the Notification of Event form to report group travel also.

In case of an incident, injury to an insured dancer (a dancer who is a member of an insured club), or a liability claim, report the incident as soon as possible.  Use the Club Accident Report form to report the incident.  After the Club Accident Report is received, you will be sent a claim form to fill out. 

There is a new form to request coverage when having a Special Dance.  The instructions for filling out the form are at the top, then mail the completed form to the address below.
